New Patients can make an appointment request by contacting reception on (07) 3831 9322, or you can make a realtime booking after gaining access to the Patient Portal.
All patients should ensure your medical history is up-to-date.
A referral from your GP is desirable but not essential. Please be advised you will not be eligible for a Medicare rebate if you do not have one.
Please submit the New Patient form below. After we receive your details, we will create a Patient Portal account for you.
When the account is created you will receive an email message to the address provided, confirming your login credentials. Please use these credentials to login to the Patient Portal.
Once you have access to your Patient Portal account, please do the following:
- Review to ensure your information is complete and up-to-date.
- Make an appointment via the real time booking system.
